Medical Cannabis Program Print and Mail Applications

You will need the following materials:

• A completed application, including:
  • Patient Application
  • Health Care Professional Verification Form
  • Caregiver Application (if applicable)
• A $50 check or money order, made out to the Vermont Medical Cannabis Program
  • To make a cash payment, email the medical program (CCB.Med@vermont.gov) to schedule an appointment.
  • The CCB may not be able to accommodate walk-in cash payments.
• Access to a working email account to submit your digital photo
  • Printed photos will not be accepted.
  • A photo of your driver's license will not be accepted.
• A valid Vermont driver's license or non-driver ID
