Print & Mail Medical Cannabis Application Materials

If you have started an online application, do not submit a printed patient application. Submitting materials both online through our portal and via mail may result in a delay in application processing.

After signing up, please use the button at the top of the page (access your exiting account) to sign in. Your username will be your email.

What do I need to complete my online application?

  • Access to a working email account
  • A digital photo of yourself
    • Photo should be front-facing, head and shoulders only. No caps/hats, sunglasses, or masks.
    • Photo must be clear, in color, and reflect your current appearance. 
    • Photo is of ONLY YOU. There should be no one else in the photo.
    • A photograph of your license will not be accepted.
    • If the photo you upload looks distorted or is oriented incorrectly-we can fix this on our end. 
  • A credit or debit card for the $50 non-refundable fee
    • Check, money order, or cash payment NOT ACCEPTED for online applications.
  • A valid Vermont's driver's license, non-driver ID, or placard number.
  • Prior to submitting your online application, you or your provider will need to submit your health care professional verification form. If your provider has an online portal through our website, they can complete and upload the form, or they can choose to print the from our website and fax, email, or mail it. If we have not received your healthcare form prior to your application, you will be issued an incomplete form and will need to rectify this within 30 days.
